Tyler Perry’s Sistas | Season 9 Episode 2 Review: Do No Harm
Phenomenal.
That’s honestly the best word to sum up Sistas Season 9, Episode 2: Do No Harm. The writers and actors absolutely delivered. This episode was emotional, intense, and real.
The only misstep? The whole Sabrina-Rich mess. I know, it was just a short convo between her and Maurice on the phone, but I can already tell — this storyline is going to be one of my least favorites this season. It feels like Sabrina is about to go through a “Danny arc.” Y’know, that Hot Girl Summer nonsense from back in Season 5 or 6? Yeah… I’ll save that rant for another day.
But let’s focus on what worked — and wow, this episode worked.

Now, let’s talk about Karen’s storyline — and how the frustration we’ve had for seasons with her has now shifted toward the hospital system. Nurse Janet and Dr. Cruz? Unacceptable. The neglect she’s facing in the hospital felt all too real.
This episode had major emotional weight. I’m giving it a 9/10 — one point knocked off for the Sabrina-Rich nonsense, but otherwise, it was brilliant from start to finish.
The promo for next week’s “Having Faith” looks 🔥 — the drama is not slowing down anytime soon.
Let’s get into the breakdown.
The whole episode (besides the opening traffic scene) happens in or around the hospital — mostly Karen’s room and the waiting area. And I have a feeling we’re gonna be stuck in that hospital for a while.
Andy and Zach are stuck in traffic, frustrated. Zach’s worried. The voicemail from Fatima didn’t sound right. He knows her well — it takes a lot for her voice to sound that shaky. Andy’s brushing it off, but Zach isn’t wrong here.
Meanwhile, Sabrina’s in traffic too — on the phone with Maurice — annoyed, stressed, and then… BAM! Turns out Danny was the car behind her laying on the horn the whole time 😂 They laugh it off, but then Sabrina brings up Rich. Apparently, he’s still blowing up her phone, but she’s done.
She suspects Rich and Penelope were a thing even before the breakup. Honestly? I’m over it. Sabrina’s character has been using men for what they offer — Calvin for emotional support, Jacobe for sex, Bio for bail money… So now she’s complaining? Girl. Please.
Back at the hospital, Karen’s in bad shape. She’s vomiting, bleeding — and no doctor is paying attention. Fatima updates Zach and Andy as soon as they arrive. She’s livid that Karen’s doctor is MIA. Turns out the hospital’s dealing with rolling blackouts, short staffing, and overall chaos.
Nurse Janet is rude as hell — just like in the previous episode — and nobody’s come to check on Karen for two hours. Luckily, Fatima finds another nurse (the helpful one), who admits that certain demographics don’t get proper attention at this hospital. It’s heartbreaking, but a very real reflection of systemic bias.
Then enters Dr. Vaughn, who actually cares. He checks on Karen, does an ultrasound, and suddenly goes pale. Karen asks to see her babies. He hesitates… then leaves to talk to Dr. Cruz. That’s when Sabrina overhears a horrifying conversation:
There’s only one heartbeat.
One of the twins is gone.
If they don’t act fast, Karen and the other baby could die too.
Dr. Cruz, of course, doesn’t care. He’s more concerned about Dr. Vaughn overstepping his authority. Disgusting.
Sabrina — completely shaken — bursts into the waiting room, yelling about what she heard. Everyone reacts differently. Some think she should’ve calmed down, but let’s be real — who wouldn’t freak out hearing that kind of news?
Zach overhears.
Fatima breaks the news to him: one baby didn’t make it. He breaks down. It’s raw and powerful.
Andy tries to arrange for Karen to be moved to another hospital, but Sabrina points out the obvious — rolling blackouts mean that’s not possible. The emotions are running high. Everyone’s trying to help in their own way, but the options are limited.
Meanwhile, Dr. Vaughn explains to Andy and Sabrina that he legally can’t override Dr. Cruz unless Karen gives verbal consent to switch doctors. He could lose his job, his license. The HIPAA rules are clear.
Andy — ever the attorney — takes this seriously. She calms everyone down and shifts into action mode.
Karen wakes up.
She’s fragile, terrified. Sabrina and Andy talk to her gently. Karen asks them to promise something heartbreaking:
“If I don’t make it, take care of my baby. And if it comes down to choosing me or the babies… choose them.”
Then — in the middle of this emotional plea — Karen flatlines.
End of episode.
This scene was gut-wrenching. Reading spoilers on BET.com last week didn’t prepare me for this moment. I thought she’d flatline during the C-section — nope. They did it with her girls right there. Imagine if Zach had been in that room? Whew. He would’ve torn the place apart.
Final thoughts:
Absolutely stellar acting all around. The writing was on point. The hospital scenes were intense and frustrating — but that’s the point. It felt real. And now, we wait for next week.
